Dolf mentale problemen? Huh?

Ja, na ruim 4 jaar ziek zijn, komen er toch nog mentale problemen bij mij. De trouwe lezers van het blog (en dat zijn er echt heel veel) zullen wel gemerkt hebben dat de toonzetting de laatste maanden minder positief is geworden. Zoals al eerder geschreven is dit blog een weergave van ons leven met kanker, positief maar ook negatief, en nu zit het tegen.
Al die jaren heb ik ondanks de grote problemen nergens mee gezeten. Het glas was altijd half vol en nu ineens is het half leeg. Of, zoals Marjolein het zo mooi verwoordde, altijd zag je nog wat je wel kon, en nu ineens zie je wat je niet meer kunt.
Medio december was ik er ineens klaar mee. Al sinds februari ga ik langzaam achteruit, voel me steeds beroerder worden en kan steeds minder. 
Natuurlijk zou ik best weer gezond willen zijn, maar dat is een onhaalbaar doel, maar wat ik nu kan, is in mijn ogen te minimaal. Ik kan 4 uurtjes per dag iets doen en lig de rest van de dag uitgeput op bed of bank. Vaak krijg ik te horen van “de buitenwereld” dat men een ander beeld heeft. Dat is ook wel begrijpelijk, want men ziet mij op de goede momenten en denkt vaak dat ik dit lang kan volhouden. In werkelijkheid lig ik vaak voor of na een ontmoeting/bezoek te rusten om weer bij te tanken. 
Wat is nou wel haalbaar en geeft mij voldoende voldoening? Dat doel ligt niet eens zo ver weg. Kijkend naar waar ik vandaan kom met een levensbedreigende bacterie, acute leukemie, een stamceltransplantatie met grote complicaties, beademing en reanimatie mag ik blij zijn dat ik er nog ben, en dat ben ik ook! Graag zou ik naast wat ik nu kan, nog stukjes willen wandelen of een rondje fietsen. Als ik die doelen haal, ben ik tevreden.

Maar zover is het nog niet en zoals genoemd, ik zit er doorheen. In 2015 heb ik precies 52 polibezoeken gehad (inderdaad gemiddeld eentje per week). Daarnaast ook nog een opname in het ziekenhuis van een week en veel bezoeken aan mondhygiëniste, tandarts, fysio en sportfysio. 


Medio december was ik er helemaal klaar mee. Ondanks alle inspanningen gaat het alleen maar achteruit. Twaalf maal aderlaten (halve liter bloed af tappen) en nog steeds een ferritinenivo dat 100-200x te hoog is en niet wil zakken, compleet uitgeput en weer lichte afstotingsverschijnselen in mijn mond. Tot grote schrik van alles en iedereen direct besloten om te stoppen met alle medicatie en behandelingen. De geplande aderlating heb ik afgezegd. Vorige week hebben Marjolein en ik deze nieuwe situatie met de huisarts besproken. 

Mijn hematoloog, waar ik mijn leven aan heb te danken en die absoluut alles uit de kast haalt om mijn problemen op te lossen, wilde ik het mondeling vertellen. Afgelopen maandag was het zover en hadden we weer een geplande afspraak. Uiteraard zoals altijd 45 minuten van te voren bloed geprikt als controle. En ja, ik zag heel erg tegen dit gesprek op. Een beter hematoloog kan men zich niet wensen. Deskundig, hartelijk en communicatief vaardig.
Hij begint direct na binnenkomst over mijn Hb. Die is met 9.6 hoog, dus hij vraagt of ik wel heb adergelaten, 2 weken geleden. Ik vertel hem het hele verhaal, en hij begrijpt natuurlijk gelijk wat er aan de hand is, zeker omdat ik hem aangeef dat het vooral bij mij mentaal is omdat het einddoel, een beetje fietsen en wandelen, zo ver weg blijft. 
Snel komt hij met een aantal punten om de problemen op te lossen. Direct geeft hij aan dat het echt zal verbeteren en dat het hoogst waarschijnlijk is dat het hoge ferritine gehalte (de zogenaamde ijzerstapeling), naast mogelijk de zware chemokuren, de boosdoener is. Als eerste verwijst hij me door naar de revalidatiearts. Dit is een arts die breder kijkt naar alle problemen, dus niet alleen naar bloed en kanker, maar naar het grote geheel. Ten tweede maken we een afspraak bij maatschappelijk werk van hematologie/oncologie. Toen ik anderhalf jaar geleden 4 maanden in het ziekenhuis lag, heb ik ook een keer gezellig met hem gesproken, maar toen had ik mentaal geen problemen, nu wel. Ten derde spreken we af dat de hematoloog gaat kijken waar ik “ferese” kan laten doen. Dit is een soort dialyse waarmee ze ferritine uit het bloed kunnen halen. Dit doen ze niet in het UMCU en moet dus in een ander ziekenhuis gebeuren. Voordeel van ferese is dat het 2 tot 3 maal sneller gaat dan aderlaten. Ik verwacht dat het anders zeker nog 1.5 tot 2 jaar zal duren voor het ferritine weer normaal is.
Bij thuiskomst ontvang ik een mailtje van de hematoloog. Hij heeft mijn casus nogmaals met 5 collega’s besproken en men is het met hem eens. Belangrijkste is om het ferritine (snel) omlaag te krijgen. Men heeft bedacht om wekelijks te gaan aderlaten (halve liter per keer) en over 2 weken weer een bezoek te plannen. 
Zijn reactie op het plotseling zonder overleg stoppen met de medicatie? Volgende keer wel overleggen! En dat ik nu heel veel pijn heb (rug, heupen en schouder) en last heb van maagzuur geeft aan dat de medicatie toch niet voor niets is. Hij laat het aan mij over of ik er zonder kan en is eigenlijk blij dat ik ben gestopt met de oxycodon (morfine).

No sé si será culpa del root, pero la actualización directa no es válida por un error en la ROM (la he bajado n veces y nada, siempre el mismo error). When your trying to figure out what to do with your life it can be overwhelming.It if’s any comfort, most people I know are still trying to figure out what they want to be when they grow up. Most people I know have kids in college!The economy is a mess, but when I graduated college in 1987 we were in a recession also. Things will work out. They always do.
